Our Programmes

Nine thematic areas shape where MED25 is going between 2026 and 2030. They are our strategic direction, not services running today.

These themes are the programming architecture set out in our Strategic Framework 2026-2030. We are deliberately phasing them rather than launching all nine at once, so that each is delivered properly. What runs today is listed under Health Services and Our Work.

What makes our approach distinctive

Each theme carries a community intervention designed for the realities of farming and fishing communities on Lake Victoria.

  1. Planned 2026-2030

    Community Primary Healthcare & Health Systems Strengthening

    Improve equitable access to quality, affordable, people-centred primary healthcare for underserved communities.

    Mobile outreach to the islands

    Outreach teams carry essential services to Lake Victoria islands and remote fishing camps, while a community emergency transport system links isolated households to care.

  2. Planned 2026-2030

    Maternal, Newborn, Child, Adolescent & Reproductive Health

    Reduce preventable morbidity and mortality among mothers, newborns, and children across Mbita and surrounding communities.

    Father Champions Network

    Respected male fishermen, boda riders, teachers, and faith leaders trained as maternal health ambassadors, driving male involvement in antenatal care and skilled delivery.

  3. Planned 2026-2030

    HIV, TB, NTDs & Infectious Disease Control

    Reduce disease burden through prevention, early detection, treatment adherence, and community systems strengthening.

    Beach Health Surveillance Model

    Community Health Volunteers stationed at fishing beaches monitor infectious disease risk, sexual health, and substance use, linking vulnerable fishing populations to care.

    Non-Communicable Diseases, Mental Health & Healthy Ageing

    Reduce premature mortality and disability from preventable NCDs and mental health conditions in rural communities.

    Village Wellness Clubs

    Community-led groups combining blood pressure checks, nutrition sessions, mental wellness activities, exercise, and peer support.

  5. Planned 2026-2030

    Nutrition, Food Security & Climate-Smart Community Health

    Improve nutrition outcomes and livelihood resilience among vulnerable households in the Lake Victoria region.

    Prescription Kitchen Gardens

    Patients presenting with malnutrition, diabetes, or hypertension receive seeds, CHV follow-up visits, and tailored nutrition coaching.

  6. Planned 2026-2030

    Water, Sanitation, Hygiene & Environmental Health

    Improve environmental determinants of health and reduce water- and sanitation-related disease burden.

    Healthy Village Competition

    Villages compete quarterly on sanitation coverage, immunization rates, antenatal attendance, clean water access, and school attendance.

  7. Planned 2026-2030

    Youth Empowerment, Gender Equality & Social Inclusion

    Strengthen protective environments for adolescents, women, and vulnerable populations across Mbita sub-county.

    Health-to-Wealth Youth Clubs

    Health literacy, entrepreneurship, climate action, mental wellness, and life skills for young people across Mbita fishing communities.

  8. Phase 3, years 3-5

    Digital Health, Research, Learning & Innovation

    Leverage data, technology, and evidence for improved community health outcomes and organizational learning.

    Community Health Innovation Lab

    Piloting, documenting, and scaling new community health delivery models designed with and for Mbita communities.

  9. Phase 3, years 3-5

    Emergency Response, Humanitarian Health & Community Resilience

    Strengthen community preparedness and resilience to emergencies and public health threats in the Lake Victoria region.

    Lake Victoria Resilience Hub

    MED25 as the lead emergency health coordination point for Mbita sub-county, integrating flood, outbreak, and climate shock response.
